However, ideally, drugs should be prescribed and used in exact accordance with the best understanding of their appropriateness for the particular patient taking their benefit / effectiveness and risk into account. Yet, mistakes in prescribing and handling of drugs may occur and patient safety may be at risk. Moreover, despite that the drugs are prescribed and used correctly, patients may experience an adverse drug reaction. Adverse drug reaction constitute a significant health problem with deleterious consequences for the patient as well as for society. Suspected ADRs have been reported to occur in about 2- 14% of hospitalized patients. When a pharmaceutical drug is approved for marketing, the drug is tested only on a limited number of patients for a limited time period in a controlled environment.
