Stephen L. Isaacs is president of Health Policy Associates in San Francisco, California. James R. Knickman was vice president for research and evaluation at The Robert Wood Johnson Foundation in Princeton, New Jersey at the time this book was written. He is currently the president and chief executive officer of the New York State Health Foundation.
