A theory generative design was used as illustrated by Chinn and Kramer's (1995). These steps are analysis & identification of the concepts; construction of relationship statements; model description; evaluation of the model. The following central concepts were ascertaining namely management; partnership and environment as basis for a model development. It was concluded that the facilitation process by the registered could be done in five Phases namely situational analysis; establishment of partnerships; management process; maintaining conducive environment and termination & evaluation. The model in this book can be used by the nurses or other health professionals and community health workers and PhD student.
