Access Day: Building a Framework for Rural Health Equity in New York State is Volume 1 of the Rural Equity Blueprint Series (REBS) published by The SozoRock Foundation, Albany, New York. The Rural Equity Blueprint Series establishes a practical model for strengthening readiness, literacy, and access equity across rural and underserved communities. Volume 1 outlines the Foundation's Access Day framework-a scalable approach counties and institutions can adopt to improve patient access, preventive engagement, and community coordination. The report synthesizes field insights, operational data, and systems-level observations to guide local planning, support workforce alignment, and inform forthcoming pilots such as Access Day and the Library Health Equity Hub. The analysis, structure, and modeling presented in this volume were developed independently by The SozoRock Foundation. This publication addresses the structural causes of rural health inequities through a governance-based model that integrates technology, workforce renewal, and performance accountability. Drawing on regional data, workforce assessments, and SozoRock® internal modeling, the framework demonstrates how coordinated governance and measurable system design can strengthen service delivery in medically underserved areas. The report establishes a replicable prototype-Access Day-as a governance-based mechanism for aligning patient engagement, academic participation, and public-health accountability. Using the SozoRock® Equity and Resource Platform (SERP) and Rural Equity System (SRES) architecture, it introduces a standards-based model for counties, providers, and universities to transition from episodic outreach to measurable system integration. Autor

Oluwabiyi Adeyemo is a strategic management professional and modernization analyst whose work strengthens governance, health-systems readiness, and administrative intelligence across the United States and Canada. He serves as Director of Strategic Initiatives for The SozoRock Foundation, where he leads national and binational initiatives advancing data-driven modernization, workforce intelligence, and resilient public-sector systems.His professional experience includes advisory and analytical roles that span health equity frameworks, digital compliance, fiscal governance, and modernization design for rural and mid-size jurisdictions. He previously led strategy and product initiatives through his consulting firm in the EMEA region and later worked within JPMorgan's Corporate & Investment Bank supporting Fortune-level institutional clients.Adeyemo earned his first undergraduate degree in Finance before completing an MBA from the University of Roehampton in London. After several years in consulting, he pursued a second undergraduate degree in Information Technology Management at SUNY Delhi, graduating summa cum laude. He is currently a Doctor of Business Administration candidate in Strategic Management at Liberty University.His publications, including the Rural Equity Blueprint Series and UnReady to Lead: How America's Leaders Are Built, contribute to the national conversation on systems resilience, governance capability, and institutional performance. His work is used by policymakers, academic programs, county leaders, and cross-border modernization teams seeking structured, scalable approaches to administrative reform.
