In this context, we propose to analyze the modes of failure of the circuit of these drugs in our hospital and to propose a plan of action.A multi-disciplinary working group was recruited to analyze the STD circuit and identify failures using the failure modes, effects and criticality analysis method. After validation of the process mapping, the failure modes, their effects and causes were defined during brainstorming meetings. Failure modes were classified according to criticality index. The most critical failures were The proposed action plan would enable 71% of the risks to be controlled. Three lines of action were proposed: raising awareness among healthcare staff, acquisition of equipment, and documentation of the STD circuit process.
