DRP's are frequent and may result in reduced quality of life and even morbidity and mortality. This book focused on problem relating to complication of drug therapy resulting in significant illness and hospital admissions. Several broad recommendations for the reduction of DRP's have been proposed such as improving communication between acute and ambulatory healthcare providers when patient transition between care settings, conducting regular review of prescription medications to avoid therapeutic duplication and to discontinue unnecessary drugs. Community Pharmacist can detect and solve the numerous DRP's during regular drug dispensing and counseling. It is likely that patient, especially those taken multiple medications or high risk medicines, often have more DRP's than the other Pharmacist is able toidentify at the time of dispensing. The solution to this problem is enhanced collaborative efforts among patients, physicians, pharmacists and caregivers with in the community and hospital.
