Ankle instability is a complex pathological entity. Dislocation of the fibular tendons is one of the causes of this instability. Ankle instability is a complex pathological entity. Dislocation of the fibular tendons is one of the causes of this instability. It is defined as the permanent or intermittent displacement of the tendons from the retromalleolar groove on the lateral surface of the lateral malleolus. It is a rare but not exceptional lesion. In this work, we report a case of fibular tendon dislocation, surgically treated in a patient, a leisure sportsman, who presented with ankle instability accompanied by clicking on the lateral surface of the ankle, forcing him to stop playing sports. 4 tunnels were drilled from back to front in the lateral malleolus. The periosteum was applied to the posterolateral edge of the malleolus with a non-absorbable nylon thread, thus eliminating the periosteal detachment, and the peroneal tendons were replaced and the sheath closed with separate stitches.In the literature Endoscopic repair, a promising procedure, has recently produced good results.
