The author was driven to write this book for several reasons. It is his belief, based on personal experience, that the subject matter, a common phenomenon, is largely ignored at worst or badly taught at best with predictably bad or uncertain outcomes. He believes that his message needs to reach surgical colleagues and that many will benefit.

Autor

The author was a practicing board certified general and colorectal surgeon in private practice for twenty years in Johannesburg, South Africa, and subsequently in Houston, Texas, for twenty one years before retirement. He is certified by examination by the Royal College of Surgeons of England and by recognition by the American College of Surgeons. He had eleven years of surgical training in both South Africa and the United Kingdom. He was a part-time tutor in surgery and anatomy in the University of Witwatersrand, Johannesburg, and also the University of Texas in Houston.
